Has anyone who has filled out the application in the past not provided a judge's e-mail address? I feel a bit awkward asking a federal judge for his e-mail address, and his clerk said I could provide hers (she volunteered that). Do you think that is acceptable?

I suggest that when you ask him to be a reference, and he agrees, you tell him they want an email address and ask if you should use his clerk's or some other email address. Then go with whatever he says. About what phone number to give them, you can ask if you should use the chambers number or if there's another that he would prefer.
If you are past that point, just use the clerk's email as the clerk suggested.

I had a number of federal judges as references. Some didn't mind their own email address, some sent it to their secretary/judicial assistant. Just ask, "They requested an email address. What would you prefer for that?" They will tell you.

I have a US Circuit judge as one of mine and, per her instructions, I use her assistant's email. It's always been fine.

I was filling out the employment section and discovered two legal temp agencies I worked for went under and a consulting business I did a short stint at was bought out. All three were over 10 years ago, but I have no way of locating the people I worked with. So - I guess that section will be a little light in the way of references.
I've been at my current job for over 10 years, but have had different supervisors through promotion, etc. I'm just listing my current supervisor, as some of my other supervisors will be listed as professional references. Anyone see an issue with that? I'm not listing each individual position in the employment section, but it is in my resume....
