For former employers, do we also provide them with contact info?
My last supervisor at the govt agency where I worked had a personal problem with me for no other reason than her own insecurities. That's why I left. And when I resigned she retaliated against me on my last evaluation. There is no way I will voluntarily let anyone talk to her. Anyway, she only supervised me for a short time. The manager who knows my work best is one of my references but she is no longer at the agency. Will I be able to specify that she should be contacted?
In light of what PF has said we can't take any chances on a wildcard reference or former employer.

On an SSA background, you provide the names of the people they contact. If you worked somewhere eons ago and your old supervisor is gone, you can put someone else in. In your situation you can certainly put in the person who was your manager as a contact, but if you do, you can't use them as a reference. I've put down people who I used to work with who got promoted after I left to the position of supervisor, although not mine. I also worked for a year at a law firm a long time ago that is no longer in existence - I'm not going to track any of those people down for the background info.
If someone is going to kill your chances you have to do something other than put that person's name in the background info.
One other neat thing they do is ask for adversaries. Well, most of them are not friendly because we meet them while calling their clients lying, cheating, sneaks. Unless you have a focused practice where you and the domestic or criminal or whatever bar are routinely on opposite sides, these people are not that friendly. Even when you have a decent relationship with them, it would be hard to translate that into any really positive recommendation for a job. I found out after the fact that you can put in regular references rather than adversaries if you want, which might be a good alternative as the adversaries have never done me any real favors.
