I vaguely recall reading a thread more than a year ago on this board or the other board about a SSA OIG investigation. As I recall, in some hearing office or region decision writers were required or expected to insert or incorporate testimonial evidence into the decisions that didn't actually exist. I think the testimony was medical or vocational in nature.
Apparently this was a widespread practice over a period of time within an office or region. As I recall the Agency even had the audacity to pursue disciplinary action against some of the writers involved. If I remember correctly, the writers had the Nuremberg defense -- they were only following orders.
Can anyone shed more light on this matter? Does anyone know of the location of the SSA OIG report if there was one? (E.g. a hyperlink to the report via the web) Does anyone know which hearing office(s) or region(s) was (were) involved?
Lastly, does anyone know if any of those involved (e.g. Group Sups, HODs, AAs, SAs, etc.) became SSA ALJs subsequent to February 1, 2008?
