I can't get the #$%^& form marked "SSA Applicant Information Omni" to open! The other four opened up? Anyone with the same problem??

Not 100% sure, but I believe Omni is in a new Word format (.docx). I would suggest sending it to someone with a newer computer or contacting ODAR and asking for an older version.

It's MS Word 2007 format. If you have older MS Word, there are free converters available you can download from the MS website. Otherwise take it to someone (or a place like Kinkos) whose computer has Word 2007 (waiting for SSA to respond might be a while).

When you try to open the file, Word should offer to seek a converter download. As long as your internet connection is up, Word should do the rest.

Note that when you do open it, if it's like mine, the very first line will ask for "Last Name_____________ Last Name_____________ Middle Name_______________." Makes one wonder if accuracy for the applicant counts?

This is my very first post after lurking for a long time . . . I managed to open the form but I am surprised and kind of intimidated by the contents! It makes the bar application in my state look almost cursory. I'm assuming that when the OMNI info form asks for "ALL EMPLOYMENT SINCE YOU BECAME LICENSED TO PRACTICE LAW" it means just that . . . so, I should list the restaurant job that I had just out of law school as well as the temp work. There is no way, even with hypnosis, that I'm going to be able to remember the names of my supervisors for those relatively (and thankfully) short-lived jobs. Will that be a red flag?

I noticed the last item on Personal History Form asks the the "Name of at least 3 written items that you authored and that were published." Is this new? What is the purpose of it?

I hear ya, Eagle, about the three written and published items. All I have to offer is some embarrassing poetry and short stories from my angst-ridden teen years. Probably not what they have in mind and certainly not anything I'd voluntarily own up to!

The three written and published items was a question on last year's form too. I truthfully put the word "none" in that block, and still managed to get selected.

Most folks are not going to have three written and published items. Don't stress out over it. Although it seems like this year's OMNI form is far more comprehensive than last year's.

I understand that in the 'old days' you had to provide writing samples. That would seem to make more sense than asking for published items.

They want the Fair Credit Reporting rights faxed back to them, unsigned?? Yes. That's what the instructions say. Perhaps they just want to make sure that you got it.
